Overview

People receiving Community Habilitation services work alongside their Interdisciplinary Team to identify meaningful, person-centered goals that reflect their interests, strengths, and aspirations. Direct Support Professionals (DSPs) provide individualized support and encouragement, helping individuals build skills, gain confidence, and increase independence in areas such as personal safety, social connections, money management, daily living skills, healthy lifestyles, and community engagement.

  • Community Habilitation services can be provided in the home and throughout the community, offering support in real-life settings where skills can be learned and practiced.
  • Services may be delivered individually or in group settings, based on each person's goals, preferences, and interests.
  • Supports are tailored to each individual's unique needs and are designed to promote personal growth, independence, and meaningful participation in everyday life.
  • Progress is regularly reviewed, and supports are adjusted as needed to ensure continued success and achievement of personal goals.
  • Through skill-building, community involvement, and enriching experiences, individuals have opportunities to strengthen their independence, build confidence, and foster a greater sense of connection and belonging.

What Can Community Habilitation Do For Me?

  • Community Habilitation is a person-centered service that focuses on your unique goals, interests, strengths, and aspirations.
  • You play the leading role in identifying what you want to work on and achieve. While your Interdisciplinary Team can offer guidance and support, your voice and choices are at the center of the planning process.
  • The service is designed to help you learn new skills, strengthen existing abilities, increase independence, and build confidence in daily life.
  • Community Habilitation also encourages active participation in the community, helping you explore new opportunities, develop meaningful relationships, and foster a greater sense of connection and belonging.

Enrollment Requirements

To enroll in services, individuals must meet the following requirements:

  • Be eligible for services through the New York State Office for People With Developmental Disabilities (OPWDD).
  • Have active Medicaid coverage.
  • Have Home and Community-Based Services (HCBS) Waiver approval, if required for the service.
  • Receive OPWDD approval for the specific service being requested.

Once eligibility and required approvals have been verified, our team will guide you through the enrollment process and help coordinate a seamless start to services.
