Community Habilitation
Community Habilitation is a goal-oriented service designed by the individual to help them increase independence while building skills. This service can be provided within the home or community setting. Children and Adults are eligible for Community Habilitation with OPWDD eligibility.
Overview
People enrolled in Community Habilitation work with their Care Team to establish meaningful goals for items they would like to work on with a hired support staff. Direct Support Professionals (DSPs) assist with goals such as safety, socialization, money management, independent living, exercise, and community exploration.

Community Habilitation can be provided in both the home and community setting.

Community Habilitation can be provided in ratios 1:1, 1:2, and 1:3 based on the needs and goals being addressed.

Goal progression is monitored and adjustments are made as needed to ensure the individuals needs are met. Skill building provides both personal growth and fun experiences.
What Can Community Habilitation Do For Me?
- This is a truly person centered support service, meaning that the person enrolled will establish goals specifically meaningful to them.
- The Care Team will assist with supports as needed in determining appropriate goals, however, the member has final say!
- The purpose of this service is to learn new skills, build on pre-existing skills, help the person become more integrated within their own community, and to establish meaningful connections.
